Accessing Your Patient Record

From 1st April 2015 if you are registered with the practice for online services you will have access to a summary of your Patient Record.

 
To view key details from your Electronic Health Record follow this link and log onto SystmOnline as normal, then click the My Summary Patient Record link which should now appear on the menu on on the Home page.
 
 
The summary will include details of the following:
  • allergies
  • drug sensitivities
  • acute medication (issued in the last 12 months)
  • repeat medication (current and past)

Data Subject Access Request

Patient also have the right to access a full copy of their medical records either electronically or in paper form. Patients will need to complete a Data Subject Access Request form available at reception. Practices have 20 working days to complete this request but on many occasions these are provided before this deadline.
