Register with the surgery

We use an online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

You will be offered, at the time of registration, a new patient health check. This is an important appointment as it allows us to become aware of your healthcare needs and any medication that we regularly need to prescribe for you. Please come along to this appointment and allow us to help you. When you do attend for your check up please bring any medications, repeat lists or health information you may have with you.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 8am to 6:30pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

All patients registered at Lyme Valley Practice have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. You have the right to express a preference to see another doctor at the surgery. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice as you currently do. If you are unsure of who is your named Doctor please ask at reception.  New patients are allocated a Doctor on Registration.
